What is the Legal Status of Kava?

As a Kava grower, one of the questions I get asked most often is "What's the legal status of Kava?" Fortunately, the answer is far simpler now than it was in the very-recent past:

In the early 2000's, demand for Kava rose dramatically as it began to make its way into the mainstream. For many years, it was also being prescribed in parts of Europe to treat anxiety as well; something that the pharmaceutical companies were surely taking notice of. Shortly thereafter, reports of liver damage by a handful of people in Europe began to emerge. Unfortunately, doctors were quick to blame Kava, and soon, entire countries were banning this historically safe and effective herbal supplement. Three of those countries included the United States, Canada, and Germany. Others quickly followed suit and the legal status of kava was suddenly in serious jeopardy.

But, the health industry fought back with a vengeance. Leading that fight was the International Kava Executive Council (bet you didn't even know they existed). Even though they were outgunned by governments, pharmaceutical companies and the media, the herbal industry was able to provide more than enough evidence that the ban was unquestionably unjustified. Furthermore, the IKEC showed that the conclusions that were reached should never have been reached in the first place, as it became clear that Kava was nothing more than a convenient scapegoat for the reported liver damage. As the evidence was being submitted, ongoing research was also being conducted by a number of independent laboratories and universities behind the scenes, partly because a worldwide ban on kava kava could be devastating for several economies throughout Oceania.

Fortunately, within a small span of time, several key pieces of evidence emerged. Perhaps the most notable one came from from the University of Hawaii, as reported by the Honolulu Advertiser in early 2003. It turns out that there's a poison in the leaves and peelings (bark) of Kava called pipermethystine. And, not surprisingly, this poison could potentially lead to liver damage.

So, what's the connection?

It turns out that once the demand for Kava skyrocketed, there wasn't enough Kava root to fill the demand. Suppliers then knowingly or unknowingly bought the leaves and peelings of Kava plants (the parts that are always thrown away as waste material) from unscrupulous growers. Up until that event, the only parts of the Kava plant that were traditionally used throughout it's 3,000 year history were the roots; never the peelings or the leaves. On a related note: further research revealed that the handful of people who suffered liver damage also consumed alcohol on a regular basis, restoring Kava's place in the herbal medicine chest as a safe, effective, and pleasant herbal supplement.

Or so it seemed. Unfortunately, the damage had already been done. Countries started to lift their bans on Kava in 2003-onward, but kava was now synonymous with "but it causes liver damage." Nothing could be further from the truth, but the media had accomplished its task, and restoring Kava's reputation has been an uphill battle ever since.

So, where do we stand with the "kava legal status" question? Happily for many, the United States lifted its ban completely, Canada lifted its ban on Kava if it's imported by individuals for personal consumption, and even Germany, where much of the original controversy arose, lifted its ban on Kava in 2007. Except for a handful of countries such as Norway, Australia, and Sweden, Kava is indeed legal throughout the world once again! The Canadian Health Ministry even gave permission to export Kava to Canada, provided it was only to private individuals.

Now that's an exciting victory for Kava!

Kava Kava can now once again be enjoyed legally by almost anyone in the world, and consumers can take comfort in the fact that in over 3,000 years of use and many billions of kava drinks consumed, there hasn't been a single reported incident of kava liver damage from any products made solely from the roots of the plant. Always check your local laws to be certain of the legal status of Kava, but as of this writing, feel free to enjoy the many benefits of this ancient plant.

H Pylori Can Lead To Vitamin B12 Deficiency

H Pylori can cause an individual to have a Vitamin B12 deficiency. It is also a known contributor to gastritis, ulcers, and it can prevent to stomach from being able to absorb the Vitamin B12 you consume. According to the information on the PCC Natural Markets website, an infection of Helicobacter Pylori, especially in adults will lead to a deficiency of Vitamin B12.

One study has found that 56% of those with H Pylori are also anemic due to a lack of Vitamin B12. Taking care of the H Pylori has shown that blood levels improve and Vitamin B12 are higher for 40% of those who have been infected. Some other studies indicate that H Pylori and a Vitamin B12 deficiency are definitely linked. However, eliminating H Plyori doesn't always mean the body will be able to have a higher level of Vitamin B12. Therefore it is important to have it carefully monitored.

A study regarding the effects of H Pylori on Vitamin B12 levels can be found in the archives of Internal Medicine (Vol. 160, No. 9, May 8 of 2000). This study involved 138 individuals who were diagnosed with both anemia and a Vitamin B12 deficiency. Each individual in the study completed a Gastrointestinal Endoscopy to determine how severe the atrophic gastritis was. A biopsy was conducted for Campylobacter organisms and a complete medical history was documented. The diagnosis of H Plyori resulted in a combination treatment.

The study determined that H Pylori was found in 77 of the 138 patients (56%). They H Plylori infection and the anemia both improved with the assistance of Vitamin B12 supplements in 31 (40%) of the individuals. Therefore, it is reasonable to state that H Plylori is linked to a Vitamin B12 deficiency.

The results of various studies have shown that H Pylori damages the stomach cells which prevents it from successfully absorbing the Vitamin B12. Taking care of the H Pylori will help with the level of Vitamin B12 but not in every case. It is therefore essential that you have your Vitamin B12 levels checked at routine intervals if you have ever had H Pylori.

Long ago Vitamin D was link to simply bone diseases like rickets or osteoporosis. However, in today's world, it's considered to be one of the best ingredients to your overall health. Some safe and healthy tanning beds offer vitamin D.

I came across an article published by the American Journal of Clinical Nutrition, and found that it was quite liberating. This is why places like the American Academy of Pediatrics have doubled its suggestion regarding daily doses for kids. Over the long term this could keep them from developing rickets and instead promote various other health benefits.

Even when a baby is born, guidelines are telling us that children should have 400 international units per day. These are massive amounts compared to a decade ago, but it does make you realize how important it is today.

Dr. Frank of the AAP said, "Evidence has shown this could have life-long health benefits." Even though the word "could" is no guarantee, it's something we seriously have to think about adding to our daily diet. Whether you're spending proper amounts of time in the sun, taking Vitamin D supplements everyday or even taking advantage of a healthy tanning bed, these are all ways to get a healthy dose.

One thing to remember about the tanning beds is that I said, "Healthy tanning bed." There are several issues that form from the conventional styles, but we're now starting to see an increase in healthy beds that offer Vitamin D, reverse wrinkles, and even more serious things like lowering blood pressure and relieving pain from muscles.

Do UV Lights Produce Vitamin D Benefits?

With the winter months now here, I've been receiving an increased number of questions about Vitamin D. First some background on why you may be deficient in Vitamin D during the winter months. And then I'll touch specifically upon UV lights and UV light bulbs.

So let's start with some basics and get a good foundation. Vitamin D is called the "sunshine vitamin" because our body turns the suns rays into Vitamin D. During the summer months, 15-30 minutes of direct sunlight when the sun is at its highest (10:30am - 2:30pm) can result in 15,000-20,000 IU of Vitamin D produced by your body.

Let's dig a little deeper into that. The sun produces two types of rays: UVA and UVB. Now, UVA rays are the type you hear all the negativity about. They damage the skin and are also responsible for tanning. Tanning salons will use UVA bulbs in their beds (though some do also use UVB). However, vitamin D production comes from the UVB rays. When we are talking about natural sunlight, there is another big difference between UVA and UVB. Our atmosphere will actually deflect UVB rays. So, depending upon the season and the time of day, you may not get much, if any, UVB rays for Vitamin D production.

In fact, from mid-October through mid-March, the sun will be at such an angle that all the UVB rays will be deflected and not even reach the surface. That translates into zero Vitamin D production from sunlight. To get any production, the UV Index needs to be at 3.0 or above. You can find the UV Index for your area by looking at the EPA's UV Index website.

An easy way to keep track of this...if you live above 42-degrees latitude, then during the winter months the UV Index will be below 3.0 and you will not be able to absorb any UVB rays for Vitamin D production, due to the angle of the sun. 42-degrees latitude runs from the northern border of California through Chicago, IL, and over to Boston, MA. So if you live north of these locations, you aren't going to get any Vitamin D from the sun during these months.

If you are in one of these locations, then you may want to consider getting your Vitamin D through other methods, due to the plethora of benefits. My recommendation would be a pharmaceutical grade supplement, as this is the only way you can be guaranteed of receiving the full amount advertised on the bottle in a bioavailable form. Meaning, it will absorb into your body.

However, there is another method gaining in popularity. And that would be UV lights and light bulbs. Light therapy is especially popular with treating SAD (Seasonal Affective Disorder). But do these lights actually produce Vitamin D? Well, it depends. You now know your body needs UVB rays to produce Vitamin D. So it simply comes down to that. If the light you are using will emit UVB rays, then you will be able to use it effectively as an alternate to natural sunlight. It's that simple.

Nutritional Safety
 
Each of the B vitamins has different safety and usage factors:

Vitamin B1 - Easily destroyed by alcohol consumption, caffeine, stress, and smoking. Pregnant women may benefit from slightly higher levels of B1. Large doses (5,000 to 10,000 mg) can cause headaches, irritability, rapid pulse, and weakness. Vitamin B2 - Absorption or availability is decreased by the use of oral contraceptives, as well as by regular exercise and alcohol consumption. Vegetarians and the elderly may benefit from slightly higher levels of B2. A deficiency of riboflavin can cause skin disorders, anemia, light-sensitive eyes, and inflammation of the soft tissue lining around the mouth and nose. Vitamin B3 - Nicotinic acid (niacin) - People who exercise regularly, take oral contraceptives, or have a lot of stress in their lives may need slightly higher levels. A deficiency of niacin causes the disease, pellagra. More than 100mg of vitamin B3 can cause flushing, tingling, itching, headaches, nausea, diarrhea and ulcers. Vitamin B5 (Pantothenic acid) - Elderly people and those who take oral contraceptives, as well as those who smoke, or consume alcohol or caffeine may need slightly higher levels. Symptoms of deficiency may include depression and poor appetite. An individual suffering from biotin deficiency may seem lethargic, weak or easily fatigued. Hair loss may result from biotin insufficiency. Additionally in some cases of severe biotin deficiency, eczema occurs. The eczema can appear anywhere on the body but primarily targets the face. Occasionally deficiency will result in a slight swelling or inflammation of the tongue as well. Vitamin B6 - Pregnant or breastfeeding/lactating women, those who use contraceptives or hormone replacement therapy, and those who use antibiotics regularly may need slightly higher levels. B6 supplementation is also suggested for those who consume alcohol, smoke, and consume protein above recommended levels. Deficiency of pyridoxine is rare. However, pyridoxine deficiency often occurs in alcoholics. Deficiency causes skin disorders, disruption of the nervous system, confusion, poor coordination and insomnia. Pyridoxine is also called pyridoxal phosphate and pyridoxamine. More than 500mg can cause irreversible nerve damage. The nerve damage can cause impaired walking, numbness, tingling and poor sense of touch. Vitamin B7 (Biotin) - Pregnant women and those who use antibiotics on a long-term basis may need increased levels. Vitamin B9 (Folic acid) - Elderly people and pregnant women may need higher levels, as well as people who consume alcohol or have risk factors associated with heart disease. Deficiency of folic acid causes anemia, poor growth, and irritation of the mouth. Deficiency of folic acid is common in alcoholics, the elderly, and people who are malnourished. Folic Acid is also called folacin and pteroylglutamic acid. Vitamin B12 - Strict vegetarians and vegans, along with pregnant and/or lactating women, and those who consume alcohol or smoke may need increased levels. Vitamin D Deficiency Epidemic and The Relationship to Chronic Degenerative Disease

It is critical to your health and longevity to supplement with a full range of vitamins, minerals, and antioxidants each day, and included in that, it is highly recommended that you supplement with 1,000 IU of vitamin D per day or more. There is an epidemic vitamin D deficiency in the United States, as well as in many other countries. The evidence is clear that that we should be supplementing our diets with at least 1000 IU of vitamin D or more.

Obviously, 1000 IU of vitamin D is well above the US RDA of 400 IU/day. The U.S. government has long recommended this amount; however, it is blatantly clear that the government recommendations are considerably out-of-date!

The bottom line: if you ignore this information your health could be in jeopardy.

Recent research has shown a strong association between low blood levels of vitamin D (which most people are found to have, ...even among those who supplement with the government recommendation of 400 IU/day) and increased cardiovascular disease risk factors, such as high blood pressure, diabetes, and high triglycerides. (Arch of Intern Med. 2007, Jun 11;167(11):1159)

What is fascinating, and is critical for you to know is that there are well over 89 medical studies showing that high vitamin D blood levels (which can only be obtained through supplementation with more than 400 IU/day ...more in the order of 1,000 IU/day) reduce nearly all cancers; including cancers of the colon, rectum, breast, and prostate (some as much as 50%). In fact, the studies are so numerous and convincing that it is now medically undeniable that the higher one's vitamin D blood levels (by supplementing with at least 1,000 IU/day or more) the less the risk of acquiring cancer of the breast, prostate, colon, esophagus, pancreas, ovary, rectum, bladder, kidney, lung, and uterus, as well as non-Hodgkin's lymphoma and multiple myeloma! Please refer to the following medical references: Anticancer Res 2006 Jul;26(4A):2573; Cancer Res 2006 Jul 15:66(14);7361; Carcinogenesis 2006 Mar;27(3):551; Arch intern Med 2007 May 28;167(10)1050; Cancer Causes and Control 2000 Oct;11(9):847; Lancet 2001 Aug 25;358(9282):641; Cancer 2003 Mar 1:97(5):1217; J Urol 2001 Jan:165(1)253 (and about 85 other medical studies).

One recent ground-breaking double blind placebo-controlled study involving vitamin D supplementation was published this year (2008) in the American Journal of Clinical Nutrition (Am J Clin Nutr 2007 Jun;85(6):1586). In this study involving 1,180 postmenopausal women, studying the effects of administering 1,000 IU of vitamin D (with calcium) or a placebo, the researchers found that after ONLY 4 years of taking 1,000 IU of vitamin D the risk of contracting any cancer ...again, that was ANY CANCER, was reduced by 60% compared to the placebo group. This was astounding news, ...and it got even better, as we shall see.

The researchers then excluded the cancers that were diagnosed during the first year of this study, because that excluded cancers that were present before the study began (since cancers are slow to grow and eventually be diagnosed). With this, a more thorough and detailed statistical examination of the data revealed that 1,000 IU of vitamin D (plus calcium) reduced the risk of ALL CANCERS, not by just 60% as was previously shown, but by a whopping 77% compared to the placebo group!

What does this mean? If we as a society were to supplement our diets with 1,000 IU of vitamin D each day (with calcium, ....let alone other full spectrum pharmaceutical grade antioxidants, minerals, and omega-3 fatty acids), as many as three-quarters (or more) of all cancers could be prevented in just four years! The impact of this double blind, placebo-controlled study is so profound that it is clear that EVERYONE should be taking 1,000 IU of vitamin D (or more) every day! (And, that the government recommendations of 400 IU/day are now out-dated, just as the RDA of vitamin C, that of 60 mg, is out-dated, ...unless your goal is simply to prevent scurvy.)

Furthermore, vitamin D has been shown to suppress inflammation by reducing cytokines (inflammatory modulating molecules). Thus, taking at least 1,000 of vitamin D or more per day helps protect against inflammatory conditions such as rheumatoid arthritis, chronic muscle pain (fibromyalgia), congestive heart failure, diabetes, stroke, and multiple sclerosis. References: South Med J 2005 Oct;98(10):1024; ClinTer 2005 May;156(3):115; Mayo Clinic Proc 2003 Dec;78(12):1463; Diabetes Care 2005 Dec;28(12):2926; Am J Clin Nutr 2006 Apr;83(4):754.

In addition, while practicing medicine and treating patients with arthritis, fibromyaglia, and various autoimmune diseases, I discovered the effectiveness of supplementing with other safe and powerful antioxidants. For example, grape seed extract and turmeric extract (curcurmin) provide anti-inflammatory properties (COX 2 inhibition), have been extremely beneficial in my patients suffereing with chronic inflammatory diseases. Although this article is about the significance of vitamin D, I would like to make it clear that use of supplements (vitamins, minerals, antioxidants, and essential fatty acids) in proper balance and ratios provided the greatest results to my patients.

Okay, .....what about the safety of "that much" vitamin D? Vitamin D is a fat soluble vitamin, and some have expressed concern about getting "too much." How much is too much?

Frankly, most people (including doctors) are often not aware of recent ground breaking research over the past decade, let alone the double blind, placebo-controlled study published this year that put to rest what levels of vitamin D supplementation are safe. I'm not knocking doctors, because I am one, but since we as a profession have not been trained in "nutritional medicine," most doctors are not aware about the numerous studies suggesting that hundres of thousands of lives that could be spared just in the U.S. alone if every American took 1,000 IU of vitamin D per day; and I would add, in addition to a complete spectrum of balanced supplements.

So, let's address the safety concern about taking higher levels of vitamin D. Is it possible to get take too much vitamin D? The answer is YES! But, how much is too much?

Vitamin D status can be assessed by a blood test. Be careful about the lab's reference ranges, because sometimes they will compare you to the "normal population," which is DEFICIENT ... you don't want to be "normal" (which is the same as being deficient in vitamin D). Optimal levels of vitamin D3 (25-hdroxyvitamin D) should be at least in the range of 30 - 50 ng/ml, ....and possibly pushing it to 75 - 125 nmol/L.

A study was performed to see how much oral vitamin D was required to increase one's vitamin D blood levels. It was found that taking 1,000 IU of vitamin D orally only increased the average person's blood levels of vitamin D by 11.5 ng/ml. To clarify, 1000 IU of vitamin D increased blood leves (on average)by 11.5 ng/ml, ...it did not establish vitamin D at 11.5 ng/ml. The bottom line is this: A daily oral intake between 1,000 IU of vitamin D per day, and even 4000 IU per day depending upon your baseline vitamin D blood levels, appears to be ideal.

Vitamin D toxicity does not occur until blood levels reach 150 ng/ml or more; which would generally require an oral dose of 50,000 IU per day! Therefore, doses up to 10,000 IU of vitamin D per day would appear to be safe, let alone half that much (5,000 IU per day). Reference: N Eng J Med 2007 Jul 19;357(3)266.

To drive this concept home: In order to achieve an optimal serum blood level of vitamin D of approximately 60 ng/ml, a 154 pound (70 kg) person with a baseline vitamin D level of 20 ng/ml would need to take 5,000 of vitamin D per day. Knowing that 5000 IU per day of vitamin D is considered not only safe, but ideal, no one should ever be concerned with the safety of taking a mere 1,000 IU to 1,200 IU of vitamin D! In fact, one should be concerned if they are not supplementing with a full range of supplements, including at least 1000 IU of vitamin D.

Here are some interesting discoveries summarized by the authors of a review study in the July 19, 2007 edition of the New England Journal of Medicine in which they calculated the rates of various diseases affected by vitamin D status. (Reference: N Eng J Med 2007 Jul 19;357(3)266).

They found:

A 78% reduction in Type I diabetes in children taking 2,000 IU/day of vitamin D in their FIRST YEAR of life.

A 200% increase in Type I diabetes in vitamin D-deficient children.

A 33% reduction in Type II diabetes in those taking 800 IU/day plus calcium.

A 72% reduction in number of falls in elderly people taking high-dose vitamin D (well over 1,000 IU/day).

A 30-50% more cancers in vitamin D-deficient people, i.e., those taking 400 IU/day or less.

In summary, according to the medical research it is clear that you everyone should take at least 1000 IU per day of Vitamin D3, let alone high quality, broad spectrum supplements, in order to reduce the risk of heart disease and cancers.

High-Dose Vitamin D and the Decreased Incidence of Chronic Disease

It is well known that vitamin D deficiency is associated with osteoporosis, bone fracture, increased falls, muscle weakness, increased risk of certain cancers (particularly breast and colon, and possibly prostate), autoimmune diseases, obesity, insulin resistance and type 2 diabetes, schizophrenia, depression, asthma, lung dysfunction, influenza, kidney disease, high blood pressure, and cardiovascular disease. Even complications of pregnancy, preeclampsia (pregnancy-induced hypertension) and gestational diabetes, are associated with vitamin D deficiency. In infants, the vitamin's insufficiency has also been linked to low birth weight (caused by low levels from the mother during pregancy) neonatal hypocalcemia (low blood calcium), poor postnatal growth, bone fragility, and increased incidence of autoimmune diseases and childhood asthma.

Prevention of Osteoporosis

The standard recommended dose of 400 IU/day of vitamin D was in general to help maintain "good health," and to help reduce the risk of osteoporosis, falls, and hip fractures. Over the last 5 years, there has been an explosion of studies expanding our knowledge and indicating that higher doses have much more of an effect on overall health and on the prevention of various chronic diseases than just bone development and the prevention of osteoporosis. This was standard while I was practicing medicine to recommend calcium, magnesium, and vitamin D to women of menopausal age. However, I now recommend doses much higher than 400 IU/day.

Insulin Resistance

A common problem I saw in my medical practice, associated with aging and increased weight was insulin resistance, which often led to metabolic syndrome or full-blow Type 2 diabetes. To help reduce the risk of diabetes, metabolic syndrome, and insulin resistance I would encourage my patients to maintain a low-glycemic diet, exercise at least 4 times a week, and be on a broad spectrum nutritional supplementation program. Often this made the difference for people who took it seriously and made significant changes.

Insulin resistance begins with oxidation of insulin receptors. Anything that cause more oxidation, and therefore more inflammation, can lead to insulin resistance, and ultimatley Type 2 diabetes. Continued consumption of high-glycemic food (simple sugars and starches) will damage insulin receptors via inflammation/oxidation. With continued oxidation and inflammation of insulin receptors full-blown Type 2 diabetes develops. Lately, it has been discovered that vitamin D deficiency is a contributing cause to insulin resistance, giving further support to my recommending higher doses.

Although not all studies have been consistent and demonstrating a reversal of insulin resistance with vitamin D supplementation, the studies that have shown benefits in reversign insulin resistance have been the studies demonstrating blood serum 25-hydroxvitamin D (or 25-OH-D) concentrations that were at least in the range of 35 - 42 ng/ml. This may suggest that high doses may be needed to see a benefit in the prevention, and possible reversal of insulin resistance.

Decreased Risk of Breast and Colon Cancer Associated with Supplementation

As a former practicing board-certified Ob/Gyn, I had many patients who had a history of breast cancer, or a family history of breast cancer. Colon cancer was also a concern, as its incidence is second to breast cancer. A decreased risk of breast and colon cancer (and possibly prostate cancer) has been associated when blood serum levels of 25-hydroxvitamin D (25-OH-D) are 40 ng/ml or above. For most women, this would require a dailly dose near 4000 IU/day.

I had many patients using tamoxifen to treat or suppress the recurrence of breast cancer. However, tamoxifen has its own concerns (among them possible endometrial cancer). Interestingly, much like drugs that are used to treat breast cancer, such as tamoxifen, vitamin D enters breast cancer cells and triggers apoptosis; whicself-destruction, or "cellular death," of cancer cells. Actually, several various antioxidants; e.g., turmeric extract, polyphenolic compounds; such as grape seed extract, cruciferous extract, resveratrol, green tea extract, and olive extract) all cause apoptosis, or cellular self-destruction. The benefit of apoptosis compared to chemotherapy drugs and radiation therapy is that it causes cancer cell dealth without toxicity or destruction to normal cells.

Although the knowledge and use of antioxidants to kill cancer cells is as of yet limited, and thefore too early to be used to replace current toxic therapies for cancer, there is much promise for vitamins and other antioxidants in the near future. According to the research, I am a believer that antioxidants, and like substancs may help to prevent to the initiation of cancer, and therefore, along with a proper diet and lifestyle, everyone should be taking quaility supplements.

Specifc to this article, as I have searched to find out what dose of vitamin D women "should be" taking for preventative measures against breast, ovarian, and colon cancer, I have discovered that the studies have shown an association between women whose serum 25-OH-D levels of at least 52 ng/ml or higher and a 50% decreased incidence of breast cancer. Ovarian and colon cancers are similar in that there is a significant decreased incidence when 25-OH-D levels are above 50 ng/ml; and particular when they reach 80 ng/ml.

As I speak on the "Power of Nutritional Supplementation," both in lectures and on my audio CD, I suggest that among other nutrients, when one considers there are so many potential health benefits to be obtained from nutritional supplementation of specific vitamins, let alone a full-range of quality supplements, with virtually no toxic effects, I have to ask why would anyone wait to supplement?

Reduced Risk of Heart Disease Associated with Vitamin D

Vitamin D deficiency increases the risk of both ischemic and non-ischemic heart disease. Supplementation also helps control blood pressure, it influences parathyroid hormone levels, influences the function of heart muscle, and plays a role in reduction of inflammation and calcification of blood vessels, thus helping to reduce atherosclerosis.

The risk for heart disease is particularly high when 25-OH-D blood levels are below 15 ng/ml. With 25-OH-D levels above 30 ng/ml cardiac benefits may possibly be significant. Obviously, more studies are needed to draw conclusions about this vitamin's role in maintaining and/or reducing cardiovascular disease, but once again, I can only say that there is now enough evidence when taken as a whole, for everyone to consider supplementing with adequate doses of this all-important vitamin for any or "all" of the potential health benefits.

Reduced Risk of Influenza with Vitamin D

It is well established that vitamin D reduces the incidence of respiratory infections. Children who have been found to have the lowest 25-hydroxyvitamin D blood levels are 11 times more likely to develop respiratory infections. I have been asked about the safety and effectiveness of children supplementing with this vitamin. One can only look to clinical information. Children with recurrent respiratory infections have been given as high a dose as 60,000 IU of vitamin D3 (the active form) each week (for six weeks), and found to have not one such infection for the following 6 months!

Many people suffer and die from influenza, or the "flu." They don't die of the viral infection per se, as much as they die from the body's over-reaction. The influenza virus causes an uncontrolled over-production of inflammatory cytokines. Interestingly, vitamin D turns down this process by "down regulating" the expression of pro-inflammatory cytokines, such as tumor necrosis factor-alpha.

Actually, the pro-inflammatory cytokine process is the same inflammatory process that occurs in chronic disease, such as inflammation of arteries in atherosclerosis, inflammation in arthritis, damage to neurons, and inflammation and damage caused by cancer cells and by the aging process itself.

Think about the possibilities of reducing inflammation, by down regulating cytokines (tumor necrosis factor alpha, etc.) and thus reducing the incidence and progression of many chronic degenerative diseases, including heart disease, cancer, Alzheimer's disease, arthritis, diabetes, respiratory infections, liver and kidney disease, and the aging process itself!

Regarding vitamin D (really a hormone) and the protection of respiratory infections and influenza, in addition to the down regulation of inflammatory cytokines, this unique hormone also up-regulates the expression of anti-microbial peptides in immune cells. Anti-microbial peptides damage the outer lipid membranes of influenza viruses, bacteria, and fungi allowing the white blood cells (macrophages) from the immune system to eliminate them from the body.

I suggest that adults and children supplement with much higher doses than what was has been traditionally recommended to reduce the incidence of respiratory infections and help support the immune system, particularly during winter months when exposure to sunlight is decreased.

Vitmain D May Help Reduce the Risk of Complications of Pregnancy

Maintaining health during pregnancy is a chief concern of mine because it is critical that women get proper cellular nutrition throughout their pregnancies. Optimal supplementation should take place before a woman even knows she's pregnant. This is one reason why I recommend all women of child-bearing age be on a full-spectrum of quality nutritional supplements. Now, with recent information about the benefits of vitamin D during pregnancy, the amount of vitamin D in prenatal vitamins is quite inadequate,...at least in my opinion.

Vitamin D plays a key role for calcium metabolism during pregnancy and development in order to prevent infantile rickets and adult osteomalacia. During pregnancy, calcium demands rapidly increase, particularly in the third trimester. Because of this, vitamin D, which is required for calcium incorporation into the bones becomes crucial for proper skeletal growth and optimal maternal and fetal outcomes. Despite widespread use of prenatal vitamins containing what is thought to be "adequate" doses of vitamin D, there is still a vitamin D deficiency epidemic among pregnant and lactating women.

It is even more important that a pregnant woman have optimal vitamin D blood levels than a non-pregnant woman, as her baby's health depends on it,...not only during fetal life, but during the next two decades of that baby's life!

Low vitamin D levels during pregnancy and infancy are associated with maternal preeclampsia and pregnancy-induced hypertension, neonatal low birth weight and hypocalcemia, poor postnatal growth, bone fragility, and increased incidence of autoimmune diseases and asthma of the child.

During pregnancy a minimum blood level of vitamin D (25-hydroxyVitamin D) should be at least 32 ng/ml or greater; and optimally closer to 50 ng/ml. In order to achieve and maintain 25-hydroxvitamin D serum levels at 32 - 50 ng/ml, I suggest that pregnant and lactating women would possibly need to supplement with a dose between 2000 and 4000 IU/day of vitamin D3 per day, which is both safe and effective. Of course, this is beyond the level found in all prenatal vitamins.

Childhood Asthma Linked to Vitamin D Deficiency

The asthma epidemic may be explained in part by the vitamin D deficiency epidemic. There is a direct correlation between low serum vitamin D levels and higher risks for asthma exacerbations. Vitamin D plays a role in fetal lung growth and development. Epidemiologic evidence suggests that higher prenatal vitamin D intake has a protective role against wheezing illnesses in children. Vitamin D may protect against wheezing illnesses through its multiple immune effects as discussed above. In addition, vitamin D may play a therapeutic role among asthmatics from becoming steroid resistant.

Above I indicated that vitamin D plays an important role in reducing the risk of respiratory infections. In children, and adults for that matter, with asthma, supplementation with high dose vitamin D is of even more importance, as a respiratory infection can trigger attack.

Measuring Your Vitamin D Status

The question on most people's mind is, "How much vitamin D is the right dose?" The best way to know one's "correct" dose is to know your 25-hydroxyVitamin D blood level. This is the metabolically-active form of vitamin D, and is also known as calcidiol. It is abbreviated as 25-OH-D. In the past, "adequate" blood levels were set to prevent rickets and then osteoporosis. The myriad benefits and safety of vitamin D is coming to light.

Most laboratories have a normal reference range for 25-OH-D of 32 - 100 ng/ml. After studying what many of the experts in the field of vitamin D deficiency treatment have recommended, it varying widely, I am in line with suggesting that one's "optimal" serum 25-OH-D level should be between 50 - 80 ng/ml.

What Dose of Vitamin D is Optimal?

The vitamin D dose required to attain a serum level range of 50 - 80 ng/ml will vary from person to person, mostly based upon body mass (weight) and sun exposure. Although this will vary considerably, a 150 lb person who supplements with 2000 IU of vitamin D per day may attain blood level range of 25-OH-D between 30 and 45 ng/ml, depending upon sun exposure. In my experience, it seems that this same person may need to take 4000 IU/day, or more, to attain optimal blood levels between 50 and 80 ng/ml.

Again, from my experience, and it this will vary widely. A person who weighs 225 lbs. may require a dose of 10,000 IU/day to maintain 25-hydroxyVitamin D blood serum levels between 50 and 80 ng/ml. A person's true dose can only be determined by blood tests and titration (adjustments based upon the blood tests).

My suggestion would be to start with vitamin D at either 4000 to 6000 IU/day, depending upon your weight; or you may safely opt to take 10,000 IU/day, and then have your 25-OH-D blood level tested about 6 to 8 weeks later. Your optimal blood level target range for optimal health is 50 - 80 ng/ml. Therefore, once you know your blood level, you may adjust your daily vitamin D intake accordingly.

Are High Doses of Vitamin D Safe?

Is long-term "high dose" of vitamin D3 safe? There may be concern that high doses of vitamin D may elevate serum calcium levels and cause kidney stones in those at risk. There was a study in which vitamin D deficient patients received either a single oral, or a single intramuscular injection of 300,000 IU of vitamin D and followed for 12 weeks. During the 12 weeks not one person had elevated levels of serum calcium (or hypercalcemia).

This doesn't mean that chronic super high doses of vitamin D will not raise blood calcium levels; however, there are no credible reports of vitamin D toxicity with chronic daily vitamin D3 supplementation up to 10,000 IU/day, including elevation of blood calcium. In fact, many vitamin D expert clinicians are routinely recommending doses well above 10,000 IU/day. Hypercalcemia (an elevated serum calcium level) is only observed with synthetic vitamin D analogues, such as calcitriol.

Since it is safe for most people to supplement with vitamin D with doses as high as 10,000 IU/day (and possibly higher) without concern, my recommendation of a starting dose between 4000 - 6000 IU/day is quite conservative.

Are there Contraindications for High-Dose Vitamin D?

Primary hyperparathyroidism is the main contraindication. Also, high dose vitamin D3 supplementation may cause elevation of serum calcium levels in patients with sarcoidosis, tuberculosis, or lymphoma. Therefore, in such cases, patients dosing with levels above 2000 IU per day should do so only with caution AND under the care and direction of a physician.

One Last Word: Maintain Proper Ratios and Balance of Vitamins

Vitamin D3 is obviously safe. It has been underestimated as an important vitamin for many decades for maintaining optimal health at least, and possibly for reducing the risk of many chronic diseases if boosted to optimal blood levels. However, as will all vitamins, minerals, antioxidants, essential fatty acids, or any other nutrient, vitamin D should be taken in balance! All nutrients should be used to supplement a healthy diet, and used in conjunction with exercise and a healthy lifestyle.

Balance and proper ratios of nutrients are critical, as in the case of vitamin D with vitamin A. Vitamin A can neutralize the beneficial effects of vitamin D. In addition, many people are aware that high doses of vitamin A can be toxic to the liver and cause birth defects. However, most are not aware that vitamin A and vitamin D compete for each other's function in the body. Supplementing with excess amounts of vitamin A can suppress the important cancer-fighting effects of vitamin D.

Most multivitamin preparations contain vit A. Vitamin A (or pre-formed vitamin A) is different from pro-vitamin A, or beta-carotene. Beta-carotene does not interfere with vitamin D. Nor is beta-carotene associated with birth defects or liver problems.

Therefore, in choosing a quality, broad spectrum supplement brand, it is my suggestion to choose one that provides beta-carotene (a.k.a. "pro-vitamin A"), not vitamin A. This is just one of many criteria in choosing a quality supplement brand. As it relates to this article, choose a supplement that provides a daily dose of vitamin D3 of at least 2000 IU/day,...and consider taking at least 4000 IU/day and having your blood tested to achieve the target range of 50 - 80 ng/ml.

We should never rely on one vitamin, juice, or magic potion to cure all our problems. However, we should not ignore the ever-emerging evidence and promise that science is uncovering of the benefits nature has to offer in helping us to maintain optimal health.

I am a major proponent of using a broad spectrum of high quality vitamins, minerals, antioxidants, and essential fatty acids at proper doses and balance, along with healthy eating and proper lifestyle. I do not claim that all the answers are found in one vitamin or antioxidant, but many of the answers to optimal health are found in the synergistic action of the blend and balance of the fullest range of supplementation we are willing to incorporate into our lives.
